Electronic DocuSign Forms

The following forms are available to complete through DocuSign. For authentication and security purposes, a verification code will be sent to your email before accessing the form. Once you enter the code, you will be able to complete the secure form and it will be submitted directly to The United Church of Canada Benefits Centre. Group Insurance Beneficiary Designation Form: Group Benefits Beneficiary form for active members.
Pension Beneficiary Designation Form: Pension Beneficiary form for active members.
Enrol, Life Event and Dependant Form: Pension and Group Benefits Enrolment form to be filled out upon enrolment to the plan or when there is a change of information, such as changes to spouse, depending and beneficiary designations.
